CLAIM

  1. When losses occur, it is necessary to keep the scene intact, apply measures to rescue and treat human injuries and limit losses and property damage.
  2. Immediately notify to the traffic police or police agencies, local authorities to coordinate in solving accidents,
  3. Call Phu Hung immediately via Hotline 0899.888.018 to notify the loss and receive specific instructions.
  4. Within 05 days from the date of loss, Client must notify the loss to Phu Hung by document.
  5. Prepare documents according to Phu Hung's instructions

Claim dossier includes the following documents:

  1. Loss Notification and Claim Request (here);
  2. Documents related to the vehicle or driver (notarized copy or certified copy of the insurance enterprise after comparison with the original) provided by the insurance buyer or the insured:

  3. a) Vehicle registration certificate (or a certified copy of the vehicle registration certificate enclosed with the original receipt of a valid credit institution;replace the original of the vehicle registration certificate while the credit institution keeps the original vehicle registration certificate) or the vehicle ownership transfer document and the vehicle origin document (in case there is no vehicle registration certificate).
    b) Driver's license.
    c) The driver's identity card or citizen identity card or passport or other identification papers.
    d) Insurance certificate.
  4. Documents proving health or life damage (copies of medical establishments or copies certified by the insurer after comparison with the originals) provided by the buyer or the insured:
  5. a) Certificate of injury.
    b) Medical records.
    c) An extract of the death certificate or the death notice or written certification of the police agency or the assessment result of the forensic examination agency in case the victim dies on a vehicle or dies due to an accident.

  6. Documents proving damage to property provided by the buyer or the insured:
    a) Valid invoices and vouchers on the repair and replacement of property damaged by traffic accidents.
    b) Papers, invoices and vouchers related to expenses incurred by motor vehicle owners to minimize losses or to comply with instructions of the insurer.
    5. Copies of relevant documents of the police office collected by the insurer in accidents causing death to third-party and passengers, including: Notification of results’ investigation, verification and settlement of traffic accidents or Notification of investigation conclusion and settlement of traffic accidents.
    6. The inspection report to determine the causes and extent of losses made by the insurer shall be agreed upon between the insurer and the buyer and the insured.
